Palliative care at home is specialised care for people living with a serious illness. Our palliative care service focuses on relieving and preventing the suffering of clients, to improve the quality of life for them and their families, no matter the diagnosis or stage of the disease. Palliative care at home is provided by a team of specially trained people. We work together to provide the support needed, whilst enabling the client to remain in the place they feel most comfortable, their home.
Palliative care at home provides you with the one-to-one care you need to help relieve symptoms and discomfort. It improves your quality of life, comfort and resilience of patients. Our carers will work with you, your family and healthcare professionals to help with the following:
Firstpoint Homecare aim to help manage the client’s symptoms as well as promote dignity, comfort and choice. We ensure that our carers have suitable experience and offer them extra specialist training, to enable them to recognise and provide the care required for your individual needs.
To ensure that our clients receive the care they require, we work closely with the client’s family, friends, support network and all healthcare professionals involved including: District Nurses, GP’s, Macmillan Nurses, Marie Curie and other specialists. An important part of palliative care is the patient talking about their diagnosis, treatment and needs and working together with your family and Firstpoint Homecare so that we can ensure you get care that works for you.
Firstpoint Homecare has a wealth of experience supporting those living with chronic conditions and we will put a support plan in place that works for you. We know the importance of personalised care for everyone and never use a “one size fits all” approach.
